Chapter Process
1) Complete the 'Commander's Request for Mental Health Evaluation' and it must be signed by Unit Commander.

2) Fax completed form to Department of Behavioral Health 719-503-7884. For 4th ID Soldiers, take the Commander's Request for Mental Health Evaluation to your respective Embedded Behavioral Health Team.
3) Once the form is reviewed for correct completion, the Commander will be called for a future appointment for the evaluation.
