INSTRUCTIONS FOR BECOMING A CANDIDATE (PLEASE READ!!!)

|All Candidates must have an updated pair of glasses
(2 years old or less).
All Candidates must stop all contact lens wear immediately!!!!!WE CANNOT DO A PREOP UNTIL CANDIDATES ARE OUT OF THEIR CONTACTS AT LEAST 30 DAYS
All Candidates must be 21 years of age or older at the time of application (FDA Regulation).Must have 18 months remaining in the army at the time of surgery and 12 months left in same occupation and unit.
Please review the priority units. Please keep in mind that if you are not in a priority unit. The probability of having surgery decreases.
OUR WALK IN HOURS ARE:
Monday-Friday, 0800-1100 and 1300-1530
THE EYE CLINIC WILL BE CLOSED FOR TRAINING EVERY FIRST THURSDAY OF THE MONTH IN THE AFTERNOON.
Download BOTH the Commander Authorization AND the LASER Application form . Bring a signed copy of the Authorization letter and the completed LASER Application form, DURING WALK IN HOURS, to the Eye Clinic at Evans Army Community Hospital. You must be WEARING your glasses. You will then be prescreened. If you are determined to be a candidate. You will be placed on the waiting list.
