Directions
The Health Insurance Portability and Accountability Act (HIPAA) office is located on the second floor of Evans hospital (the second office to the east of the Warrior Family Medicine clinic). Patients should park in the “F” parking lot (blue signs) just north of the Woods Soldier Family Care Center; enter the courtyard entrance and turn right into the main hospital. Then go up the first “tower” staircase or take the elevator in the tower to the second floor. Turn left from the stairs or walk straight from the elevator, take a right on the open hallway to Room 2402 located on the left before you reach Warrior clinic.

Look for the sign that says “Come-On In ”.

About
Under the Health Insurance Portability and Accountability Act, the Privacy Officer is responsible for establishing and monitoring compliance controls to keep patient health information protected. Addressing confidentiality and privacy requirements, the PO oversees all activities and processes associated with creating, introducing and maintaining workplace privacy policies to meet state and federal legal requirements. The PO also monitors data access, investigates breaches and complaints -- is the primary advocate for investigations of any suspected breach of Protected Health Information (PHI)/Personally Identifiable Information (PII), and ensures all state and federal HIPAA requirements have been identified and met to keep patient/staff (PHI/PII) safe and secure. In addition, the PO manages the Freedom of Information Act (FOIA) program, ensuring accurate and timely processing of FOIA requests and serves as a subject matter expert on issues related to HIPAA privacy/security and FOIA.
