Mission, (WHAT we DO): DMC enhances the MMD mission by providing quality, safe, and timely chronic disease care in order to optimize clinical outcomes, prevent complications, and empower patients in their self-care. Vision, (WHERE are we GOING): The premier, evidence based diabetes and asthma care in the WRMC, which provides patients with cutting-edge education and management to control their disease and improve overall health and well-being. Who we are: We are a multidisciplinary team with Clinical Pharmacists, Nurse Practitioners, Registered Nurses and Medical Technicians. We coordinate care with Dieticians, Podiatrists, Ophthalmologists, Allergists, and your Primary Care Managers. What we offer: Group classes and/or individual appointments. Classes include sessions in Pre-diabetes, managing diabetes, gestational diabetes, glucometer instruction, administering insulin, managing asthma, inhaler and spacer instruction. Also included are written asthma action plans. Hours of operation: Monday – Friday 0730-1600. We are closed on holidays, training holidays, and weekends.
