The Mountain Post Medical Home is the second Army community-based primary care clinic in Colorado Springs. The clinic is located in Patriot Place just west of Powers Blvd and just off Platt Ave., at 565 Space Center Drive in the Patriot VII building. 

This new clinic operates on a patient-centered primary healthcare model using a team based, comprehensive approach to meet the patients' complete primary care and wellness needs. On-site services include primary care, pharmacy and laboratory services.

Enrollment is open to all TRICARE Prime eligible beneficiaries living within a 30 minute drive time of the clinic.

Eligible beneficiaries may choose to switch to the Mountain Post Medical Home or remain enrolled to their current military treatment facility healthcare provider.

Eligible beneficiaries who would like to change their enrollment to the Mountain Post Medical Home can do so by visiting the TRICARE Service Center located inside their Military Treatment Facility, either Evans Army Community Hospital, the 10th Medical Group, or the 21st Medical Group.

Patients can also change their PCM by filling out and signing a Primary Care Manager (PCM) Change Request Form. The form may be downloaded here or obtained at the TRICARE Service Center from 8 a.m. to 5 p.m., Monday through Friday. The TRICARE Service Center is located just inside the west entrance to Evans Army Community Hospital on Fort Carson, Bldg. 1650. Beneficiaries who need assistance completing the PCM change request should contact 1-877-988-WEST (1-877-988-9378).

After the form is completed and signed, submit it through one of the following processes:
1) Take the completed form to the Evans Army Community Hospital (Fort Carson), 21st Medical Group (Peterson AFB), or 10th Medical Group (US Air Force Academy) TRICARE Service Centers during operating hours.

*** Patients should continue to be seen at their current clinic until they receive confirmation from TRICARE that their assignment to the Mountain Post Medical Home is complete. After patients are successfully assigned to the Mountain Post Medical Home, they should continue to schedule or cancel appointments by calling 719-524-2273 (CARE).
