CORRESPONDENCE – RELEASE OF INFORMATION

1230-1630 except for Warrior in Transition (WTU) Soldiers)

Process to Request copies of Medical Records:
In order to obtain copies of your records, please come to the
Soldier Family Care Center (SFCC) in the Lower level (1st
Floor) of the new Evans Hospital addition.
Release of Information is located in Room # 1500 (the window to
the right). Please take a
number and a clip board and complete the DD Form 2870 prior to your
number being called at the window.
You will need to have a Military ID in order to complete the
request process. If you are
requesting copies of records for anyone besides yourself, please bring
your MEDICAL Power of Attorney with you.
Parents:
Please don’t wait until the day
before school begins to look for those shot records.
If you cannot locate your copy, you will need to come by the
Release of Information window to request a copy.
Copies of an entire medical record usually take approximately 30-60 days
to complete.
If you need to take a copy of any medical documentation to a Specialty
clinic appointment, contact the Release of Information Section at least
14 days prior to your appointment.
It is best to visit the ROI office to request your records prior to
making your appointment.
Depending on the referral service your medical records may have
already been sent as part of the consultation/referral process.
To find out if your records have already been forwarded to the
Network Provider, please contact this provider prior to your
appointment.
The Correspondence – Release of Information Division is the only
authorized section that can release Protected Health Information for
EACH and outlying clinics Soldiers and Family Members.
All other clinics must follow the EACH Policy 93.
In accordance with the Health Insurance Portability and Accountability
Act (HIPAA), the Release of Information Division maintains a listing of
all disclosures that have been made from your medical record (except for
those requested by you or disclosures made for billing or for your
continued health care). You may receive an accounting of these
disclosures through the Release of Information Division.
