The Clinic is located in the Woods Soldier
Family Care Center. Patients should park in the “G” parking lot,
enter the East entrance (farthest from the mountains) and take

The Evans Army Community Hospital Pediatric Clinic
will provide quality care that develops family-centered partnerships,
maximizes use of community-based health resources, ensures proper
transitions of care and brings value to our patients, their families
and our organization.
Our team members include
Physicians, Nurse Practitioners, Nurses (RN, LPN), Clinical Pharmacist,
Clinical Developmental and Behavioral Psychologist, Pediatric Nurse Case
Manager, Certified Nursing Assistants, Medics and Medical Support
Well child visits, Sick Visits (same day appointments available), Annual School / Sports
Physicals, Immunizations, Medication Renewals / Guidance, Behavioral Health for Children / Parents
Who can be seen here?
Infants and children from birth, to age 18 years assigned through Tricare Prime. Appointments
are space available, same day, after 1400 for Tricare Standard and Reserve Select.
Children will receive their scheduled immunizations during well visit appointments. Only children
who need their seasonal flu shot or subsequent HPV shots are eligible to walk in for those immunizations
without an appointment. Hours are Monday – Friday, 0800-1130 and 1300-1530. If you have questions regarding
your child’s immunizations, please contact the Pediatric Clinic.
Walk In Services Offered
- Suture / Staple Removal
- Skin testing for tuberculosis (PPD placement)
- Weight checks
- Blood pressure checks
- Drop off forms for CYS/school/sports physical, if recent well visit recorded
- Strep throat testing (For ages 3 years and older) If your child DOES NOT have a fever and sore throat,
we will not test your child for strep. Please make an appointment with a provider.

return call? Register for a Secure Message account! Parents, establish an account with your PCM first.
Then you may associate your child and your child’s PCM with your account. Services include (but not limited to)
prescription renewals, radiology results, laboratory results and non- urgent guidance from your medical team.

Q. Do I need an Appointment for a medication
A. You may send a secure message or call the front desk at 526-7653 and
leave a phone message for your provider. If you need to be seen, we will
schedule an appointment for you.
Q. Can I get a physical form filled out without an appointment?
A. If your child has a recent physical on file; your provider can fill this out for you
without an appointment. If your child is younger than 18 months, the physical must have
occurred in the past 3 months. If your child is over 18 months old, the visit must have
occurred in the past 9 months. Otherwise, please schedule an appointment with your provider.
