GENERAL QUESTIONS

Q:  I have an emergency and it is after business hours?

A:  Report to the ED, open 24 hours 7 days a week.

Q:  I need a mental health appointment for myself (active duty Soldier or service member)?

A:  Call your respective Community Behavioral Health Team if you are in 4th ID.  If not call the Department at 526-7155 and schedule an appointment.

Q:  What are the Clinic Hours?

A:  Monday thru Friday 0730hrs - 1800hrs.  Emergent walk-ins can be seen from 0730-1600.  Soldiers assigned to 4th ID can report to their respective Community Behavioral Health Team.   

Q:  Where are you located?

 

A:

Psychiatry/Psychology Services- Located on the fourth floor (east) of Evans Army Community Hospital.

Social Work Services/ Family Advocacy- Located in the Soldier Family Care Center (SFCC) of Evans Army Community Hospital.

 

Community Behavioral Health Team 1- Located in building 2650 for 1/4 BCT Soldiers.

Community Behavioral Health Team 2- Located in building 2361 for 2/4 BCT Soldiers.

Community Behavioral Health Team 3- Located in building 1657 for 3/4 BCT Soldiers.

Community Behavioral Health Team 4- Located in building 9481 for 4/4 BCT Soldiers.

Community Behavioral Health Team 5- Located in building 813 for 43rd SB, 10th CSH, and other non-divisional Soldiers.

 

PSYCHIATRY QUESTIONS

 

Q:  I need a refill on my medications?

 

A:  Simply call us at 526-7155 and we will give a message to your provider, it is helpful to have medication name and dosage amount.  The provider will contact you within 72 hours about your refill.  To ensure you do not run out of medications, please call when you have about 5 days left of your current prescription.  Thank you.

 

Q:  I have just PCS/Redeployed to Ft Carson and have been prescribed medications from another duty station/downrange.  How do I get refills?

 

A:  Go to your FMC and see your PCM.  They will be able to give you a refill and set up an appointment with your respective Behavioral Health Team for a new appointment.

 

PSYCHOLOGY QUESTIONS

 

Q:  I am scheduled to attend Drill Sergeant/Recruiter/Long Range Marksmanship school and need an evaluation.  How do I do this?

 

A:  4th ID Soldiers can go to or call their respective Community Behavioral Health Team and set up this appointment.  All others can call the Psychology Services front desk and set up the appointment.  No testing is needed for these schools.

 

Q:  I am scheduled to attend Sniper/Ranger school and need an evaluation and testing.  How do I do this?

 

A:  Call 526-7155 to schedule testing.  We will direct you to the proper Community Behavioral Health Teamor set up an appointment on the 4th Floor.

 

Q:  My unit is planning on chaptering a Soldier.  What do we need to do to get an appointment for him/her?

 

A:  First thing is to complete a Ft Carson Form 62.  It can be found in the Commander's Toolbox.  Once completed take the FC 62 to your respective Behavioral Health Team and they will set up the appointment for the Soldier.  The Soldier will need to be escorted by an NCO to the appointment.  Units cannot ask for a Chapter 5-13 or 5-17 as these are medical chapters that MUST come from the doctor here.
