Welcome to the Evans Army Community Hospital Family Practice Clinic! Our goal is to help you keep you and your family healthy. This works best if you see the same provider for your care over time. This way we get to know you better. To make this happen, you were assigned a Primary Care Manager (PCM). This person may be a doctor, nurse practitioner, or physician assistant. Your PCM is your first person to go to for care. If you have not met your PCM, we encourage you to schedule a "wellness" visit so you can get to know each other.
APPOINTMENTS & PHONE MESSAGES WITH YOUR PCM
We will try to make all your appointments, especially routine appointments with your PCM. However, this may not always be possible. To make an appointment with your PCM, call TriWest at 264-5000(select option #2). You may also leave a phone message for your PCM by calling our clinic at 524-4068 or 4069.
CHANGING YOUR PCM
If you wish to change your PCM, contact the TRICARE Service Center at 264-5000(select option #4) or stop by their office in our hospital.
APPOINTMENT INFORMATION
To help our providers do the best job for all our patients, we ask you to please:
Arrive 15 minutes before your appointment time. This helps the clerks and nurses to get you ready for your provider on time. If you arrive late, you may have to reschedule.
Do not bring children with you unless they have appointments.

Call 524-4068 or 264-5000(select option #2) at least 24 hours in advance to cancel an appointment.
Remember that most appointments are 15- 20 minutes long. When you come in, tell us first which problems are bothering you the most. That way, we can take care of important things first. If you have many problems, ask the scheduling clerk to book you a longer appointment or two separate appointments.
NURSE ADVICE LINE
Call the Nurse Advice Line (TRICARE Line for Care) at 1-888-887-4111 if you need health advice or are not sure when you need to see a doctor for your problem. It’s open 24 hours a day.
