Directions
The Managed Care Division is located on the second floor of the Main Hospital. Patients should park in the “A” parking lot (blue signs); enter the main entrance and take the first stair case staircase or set elevators right before the pharmacy to the second floor. Walk straight ahead at the top stairs or turn right from the elevators to the Managed Care Division – which is on the West end of the building.

About
The Managed Care Division serves as the liaison between Evans hospital and the Managed Care Support Contractor UnitedHealthcare Military & Veterans. The department consists of three sections: Referral Management, Nurse Case Management and Utilization Management.

Referral Management
Provides information about the status of ordered referrals, Supplemental Health Care Program claims payment status and TRICARE benefit information; it interfaces directly with UHM&V to resolve systemic and individual beneficiary issues.

Nurse Case Management
This section has a team of health care professionals who help patients and family members solve medical, educational, and social needs. NCM’s goal is to help all to reach their highest level of well-being as soon as possible. Case managers are located in the Family Medicine Clinics (Patient and Soldier Centered Medical Homes) to provide services to eligible beneficiaries

Utilization Management: Accesses the hospital’s and clinics’ processes for safety, quality, and efficiency; works with clinics and medical administrative staff to adjust processes, training or staffing to deliver better care for our beneficiaries.
