ABOUT ARMY MEDICINE

Case management teams are
groups of health professionals who help you and
your family figure out
ways to resolve your medical and social
needs. The case manager is your team "coach".
You and
the team
work out a plan to help get your optimum health
back as soon as possible. You
will also plan with them to get the right
help for what you need as you improve. The
case manager will oversee the plan to ensure
it meets any change in your health. The goal
is
to help
smooth your trip through the maze of
healthcare systems.
Who is eligible for Case management? All active duty, retirees, and active duty and retiree family members are eligible. Usually only people with very complex problems require case management. These may be medical, social, or behavioral health problems. Some examples are: Soldiers injured in Theater, cancer, very old and frail living alone, severe burns, chronic illness, chronic pain, or transplants. Will my provider be informed of these plans and services? Yes, your doctor or nurse is part of the team who helps you make decisions about planning your needs.
