Cardiac or Respiratory Arrest 

Individual in the hospital, clinic or immediate area outside the building or in the parking lot has gone into cardiac or respiratory arrest.                                                                                                

* Call *46, report location of cardiac or respiratory arrest, request the Code Blue Team. Initiate CPR. Notify attending physician.                                                         

* Follow instructions from the Code Team leader.         

* Complete Incident Report Form (e4106). 

* Medical personnel complete appropriate portion(s) of MEDCOM Form 679-R
