The Table below indicates the types of physicals we perform, lab requirements and forms to be completed for each.  Please determine which type of Physical Exam that you will be completing.

Complete the required forms, and Print a copy (YOU WILL NOT BE ABLE TO SAVE this type of template PDF)

Bring the completed form/forms with you to the appointment.

Lab Requirements

 Fasting:  Must be fasting 12-14 hours prior to lab draw.  Patient may only have WATER, no juice, COFFEE, SODA, etc.  No food, gum, or mints.  Patient can take medications, but only with water.  If you have questions, please call 719-526-7170
