  Following are the types of physicals we perform, lab requirements and forms to be completed for each.  You will receive a physical packet when you come in to get your labs done, there is no need to bring these forms with you.

*Please note that there are different requirements for different age groups!

Lab Requirements

A.  Basic and Standard:  No Fasting requirements.
B.  Fasting:  Must be fasting 12-14 hours prior to lab draw.  Patient may only have WATER, no juice, COFFEE, SODA, etc.  No food, gum, or mints.  Patient can take medications, but only with water.  If you have questions, please call 719-524-4668

2.  DD Form 2807-1 (Report of Medical History)
3.  DD Form 2697 (Report of Medical Assessment):  Required for Retirements, ETS, and Chapter  Physicals
4.  DD Form 2351 and DD Form 2492:  DOD MERB Physicals (ROTC and Academies), Green to Gold, USUHS

      Please be advised that we do not complete Overweight evaluations nor  Fit For Duty  Physical Exams
