to be completed for each. You will receive a physical packet when you come
in to get your labs done, there is no need to bring these forms with you.

Lab Requirements
A. Basic and Standard: No Fasting requirements.
B. Fasting: Must be fasting 12-14 hours prior to lab draw.
Patient may only have WATER, no juice, COFFEE, SODA, etc. No food, gum, or
mints. Patient can take medications, but only with water. If you
