With your child’s school year complete and summer break in full swing, it can be easy to overlook a few important details before school begins again in August.
Evans Army Community Hospital has you covered with our free Annual School-Sports Physical events for children ages 4-21 on July 14 and Aug. 4 from 7:30 a.m. to 4 p.m. at the Warrior Family Medicine Clinic, located on the hospital’s second floor.
“Our Evans staff members take great pride in aiding our families get ready for school and we provide this weekend clinic with an all-volunteer staff,” said Lori Burrell, Evans ACH population health nurse and event organizer. “Evans Army Community Hospital has been hosting this event for over 10 years and has found that it greatly aids our families in getting additional weekend access to healthcare during a critical time period.”
These Saturday events are intended to be a quick, convenient way for families to receive their child’s physical, which is valid for one year. Most appointments last an average of 5-10 minutes with a medical provider. Appointments are scheduled for an individual child so be sure to book one for each child in your family.
During each event, Evans ACH staff sees over 250 children so booking your appointment early is recommended to secure a space for your child.
If your child is due for shots, they will also be able to receive immunizations during their visit to the event. Parents need to bring a complete vaccination record and any school or team paperwork that needs to be completed by the medical provider to the appointment. Without these documents, the child’s approval to participate in sports or school may be delayed. Also, if your child’s school requires a specific physical form, you will need to provide it at the appointment.
With a large number of school districts in the Colorado Springs community, along with those spanning from Fountain to Monument, it is imperative to confirm specific requirements prior to the appointment due to varying guidelines.
For children who attend college and require lab testing to accompany their sports physical, please complete these requirements at the Evans ACH Laboratory prior to the appointment so the medical provider can complete the physical during the visit.
If your child is enrolled in the Exceptional Family Program or has a high-needs condition, we urge you to book an appointment with their primary care provider to complete their exam. Your child may require more allotted time than available at the event. This ensures the correct information and care plan is disseminated to their school.
Remember to bring the sponsor’s DoD I.D. card or a power of attorney along with DoD I.D. cards for children age 10 or older.
